effectiveness of TRT if you've had tinnitus for a longer period of time

Hello Dr. Nagler,

Thank you for our phone consult this week -- it was quite helpful. I did have a couple of follow-up questions that may also be of interest to your broader audience.

1) While we've agreed that TRT is not the right option for me at this time, I wouldn't rule it out down the road (like perhaps in six months or a year) if my habituation does not progress. Is TRT as effective once you've had tinnitus for a while, or is it best to start TRT soon after the onset of tinnitus?

2) There is an audiologist in my city with decades of experience who offers Neuronomics. But I believe you recommend TRT over Neuronomics. Can you give a high-level explanation behind that preference?

I'm really asking these questions on a "just in case" basis. My tinnitus is still bothersome at times, and I certainly don't love it, but I am feeling less anxious about it overall. We are sort of co-existing. I'm thinking that's a sign of progress, and hopefully a signal to my brain that the tinnitus is not worthy of its constant attention.
