Permanent spikes

Doctor, I got tinnitus 2 years ago following an ear infection. It doesn't spike, it's 1/10 pretty much all the time even though at the beginning it was 15/10. And my question is, after reading content here - is there stuff on this planet that pretty much always causes spikes and should be avoided at all costs? Basing on what I've read here I should avoid really loud noise and then it should be OK but maybe there's something else I should consider.
