Hello Dr. Nagler,
I'm working with some CBT techniques in the Wilson book, but on more challenging days I still think about TRT. There are days when a more passive approach is appealing! I have several questions that might be of interest to other forum members, as well. Here they are:
1) TRT has an 80 to 85 percent success rate. In your opinion, why do 15-20 percent fail at TRT?
2) When my T first began, I saw an ENT and an audiologist locally. The audiologist suggested using a Resound device. However, the sound generated would not be calibrated to my tinnitus. Instead, I could select a variety of soft background sounds or even soothing music of my own choosing. The audiologist said that they've learned the particular sound doesn't matter, as long as it's soothing. (Really??!!) How is that any different from listening to sound generated from apps or my own playlist music via small bluetooth devices? I am curious as to if you think there's any difference. Also, I understand this is not TRT by any stretch of the imagination. No counseling involved at all.
3) I know you highly recommend Dr. Gail Brenner out of Philadelphia for TRT. I spoke with her office manager and am impressed with how well they're equipped to deal with patients who live at a distance. I've also learned that there's a Jastreboff-trained audiologist out of Lexington, KY, which is within driving distance for me -- Dr. Ann Rhoten. Are you familiar with her?
4) I was put off from TRT initially because I thought you had to wear the devices every waking moment. Now I'm understanding it's 6-8 hours a day, which is more manageable. Do those need to be consecutive hours a day? Does it help the process to wear them for longer -- like 10-12 hours a day?
Thank you so much,
